The present study aimed to evaluate the effectiveness of doxycycline monohydrate for treating bacterial infections in South Africa. Methods: A prospective cohort study was conducted in the state of South Africa, comprising 897 children with bacterial infections diagnosed by microbiology, including a range of different pathogens. The participants had to meet the following criteria: i) being in a endemic state of respiratory tract infection (RTI) that was treated with doxycycline hyclate, a standard treatment regimen for acute bacterial sinusitis, and ii) receiving the minimum effective dose of 500 mg of doxycycline (Doxy 100 mg), the dose recommended by the WHO. The mean age of the participants was 17.8 years, with a range of 5 to 29 years. The prevalence of bacterial infections was found to be approximately 35% of all cases of RTIs in South Africa. The prevalence of resistance to doxycycline in different pathogens was evaluated using a combination of the most common pathogens, including Streptococcus pneumoniae, Haemophilus influenzae, Staphylococcus aureus, and Salmonella typhi. The most common pathogens used to treat bacterial infections were Streptococcus pyogenes, Haemophilus influenzae, Staphylococcus aureus, and Salmonella typhi. The results show that doxycycline monohydrate is effective in treating bacterial infections in children, which can be used for the treatment of RTIs in children, when used as a combination therapy. The prevalence of antibiotic resistance in different pathogens in South Africa is increasing, and it is expected to rise further in the near future.
